Please return this form with the registration fee to the St. Malachy School Office or the St. Malachy Parish Center, in an envelope clearly marked St. Malachy Athletic Committee (one form per player, per sport).  Additional forms are available on-line at www.stmalachyathletics.org or at the St. Malachy school office.
Registration is not complete unless accompanied by the appropriate check for fees.

*Virtus training is required.  A copy of the completion certificate must be on file with the Athletic Committee.  

All students are required to submit a principal release form per sport, unless attending St. Malachy school.  

All students must have a CYO physical on file with the St. Malachy Athletic Committee for the 2010 – 2011 school year.  If you do not have one on file please turn into the school or parish office Attn:  St. Malachy Athletic Registrar.*

*St. Malachy will be hosting a clinic for physicals.  The target date is during the first two weeks of 2010-2011 school year, but has not been confirmed.  The physical fee is $10 and payable at the time of the physical.
